Acknowledgements $10

A signer has been verified using an up to date identification such as a driver’s license or passport.

The signer is confirming the document is being signed willingly.

Travel Fees

$35 within Orlando City

(+ $10 for each signature)

$55 within Orange County

(+$10 for each signature needed)

Power of attorney $10

A written authorization to represent or act on another’s behalf in private affairs, business, or other legal matters.

Printing Fees

$1 per sheet

Parent consent Forms $10

Passport: If one parent or guardian cannot go with the child to apply for a passport a consent

Form DS-3053 must be filled and notarized.

Wills & Trusts $10

*Please note when traveling to a hospital the patient must be fully awake and aware otherwise notazrization will not be complete.
